Visitor Liability Release Waiver

Beak n Wings Inc. Member/ Visitor RELEASE AGREEMENT

I acknowledge that by signing this Release Agreement, and joining the Beak n Wings , Inc. Bird club as a member I am releasing Beak n Wings, Inc., and any other sponsoring or organizing parties, including, but not limited to, the property owners/management, or public entities associated with Beak n Wings, Inc., and their respective agents, directors, officials, and employees (hereafter “Released Parties”), from any and all liability for any acts, omissions or occurrences which may result, directly or indirectly, in injury to myself, my pet(s), or my personal property as a result of visiting or handling any birds owned, housed, or otherwise kept by Beak n Wings, Inc. I hereby agree to waive, release and forever discharge, on behalf of myself and my heirs, executors, administrators, legal representatives, assignees, and successors in interest, any and all claims for damages, rights, or causes of actions which I may have, or which may hereafter accrue, against the Released Parties, including any claims arising from the Released Parties’ own negligence. I further agree to indemnify and hold harmless the Released Parties from any claims, rights or causes or actions which may accrue to any individual or entity participating in, involved with, or present at Beak n Wings Inc, directly or indirectly, of my own acts or omissions. I hereby indemnify the Released Parties for any damages awarded or expenses incurred as a result of any such claims, rights or causes of action, including, but not limited to, attorney fees and court costs. I accept all liability for damage to property or persons caused by myself or my pet or any other animal in my possession. I understand that all birds are capable of biting or scratching, and can cause mild to severe bodily injury. By signing this waiver, I agree to hold Beak n Wings, Inc. harmless in any and all injuries that may be caused by birds on the property. I have had acceptable behaviors around birds explained to me by a Beak n Wings, Inc. agent, and understand that disregarding any information provided to me may result in injury to myself or my personal property. I understand that this Release Agreement is a binding contract. I have read and agree to all of its terms. This Agreement may not be modified orally, and a waiver of any aforementioned provision shall not be construed as a modification of any other provision herein or as consent to any subsequent waiver of modification.

Media Release Form

Authorization for Use of Name, Likeness, and Media Content

I. Grant of Permission,

[By signing this form I hereby grant Beak N Wings, Inc. and its affiliates, representatives, and authorized media partners the irrevocable right to use my name, likeness, image, voice, and/or video recordings in any promotional materials, including but not limited to:

• Websites and digital media

• Social media platforms

• Marketing and advertising materials

• Press releases and publications

• Events and presentations

II. Scope of Use I understand that these materials may be used for informational, educational, promotional, or marketing purposes in connection with BNW and that my participation is entirely voluntary.

III. Waiver of Rights and Compensation waive any right to inspect or approve the final use of my image, voice, or likeness, and I understand that I will not receive any compensation for their use.

IV. Release of Liability I hereby release BNW, its employees, agents, and assigns from any claims, demands, or liabilities arising out of or in connection with the use of my name, image, and media content.

V. Duration of Agreement This authorization remains in effect indefinitely unless revoked in writing. I understand that revocation will not apply to materials already published before the date of revocation.

VI. Signature & Agreement By signing below, I acknowledge that I have read and understand this media release form, and I consent to the use of my image, name, and media content as outlined above.
